All new patients should print and fill out a full set of forms, either adult or pediatric.
Additionally, please print and fill out a consent to treatment for naturopathic, acupuncture, or both, if appropriate. Please note, you will have an opportunity to ask any questions you have about the content of the consent to treat at your visit.
Naturopathic patients should also print and fill out a release of records specifically for any LABS that have been done in the last 3-5 years. It is not necessary to request your full medical record unless requested by the physician.
